REQUIRED SUPPLEMENTAL DOCUMENTATION
In order to be considered for a scholarship award, we must be able to verify income for the household. Thus, in addition to this application, we need to have this form completed and a copy of the following required documents from BOTH parents or adults residing in the same household.
If separated or divorced and you do not receive child support and/or alimony, then you can submit the required supplemental documents without including ex-spouse information.
IF YOU ARE WORKING, we will need:
1. A copy of your most recent State and Federal Income Tax Returns, and,
2. A copy of two recent pay stubs.
IF YOU ARE NOT WORKING:
Please provide proof of your source of income such as a copy of State Disability benefits, Social Security Disability Award letter, a copy of your CalWORKS, AFDC, TANF, Cash Aid documentation, proof of unemployment benefits, Social Security, and/or retirement/pension benefit check stubs.
This application will not be considered unless submitted with the required documentation.
